We Accept the Following Insurance Plans
Lott Physical Therapy is in-network with many major insurance carriers, including:
- Medicare
- United Healthcare
- Humana
- Tricare
- VA
- Medicaid (limited plans—call to verify eligibility)
If your insurance provider isn’t listed, you may still have out-of-network benefits that cover your treatment. Call us today, and we’ll be happy to check for you!
Direct Access - No Referral Needed!
Did you know? Texas law allows licensed physical therapists to evaluate AND treat you without a doctor’s referral. This means you can start physical therapy right away—no waiting, no hassle!
If your insurance requires a referral for continued treatment, we can help coordinate with your physician.
Workers’ Compensation & Auto Insurance Claims
Workers’ Compensation
If you’ve been injured on the job, we can work with your workers’ compensation insurance to cover your treatment.
To schedule your first appointment, please provide:
- Your claim number
- Your adjuster’s contact information
We will handle billing directly with your workers’ compensation provider.
Auto Insurance Claims
If your injury resulted from a car accident, you may be eligible for physical therapy coverage under your auto insurance policy.
- We accept auto insurance claims
- We accept third-party claims
- We do not accept letters of protection from attorneys

Affordable Self-Pay (Cash Rates) for Physical Therapy
If you don’t have insurance, prefer not to use it, or have a high deductible plan and would rather submit claims on your own, we offer affordable self-pay rates:
- Initial Evaluation – $150
- Extended Follow-Up Visits – $120
